A hidden failure which is already happened and not noticed by the asset operator. In the normal conditions, this type of failure become evident by itself to the asset operator unless there is another failure. The cause is identified when the root cause analysis is performed. Hence, the risk is more if hidden failures exists for longer durations and this may result in bigger losses.
